In order to facilitate engagement and increase lead conversion, editors asked to allow the EN form block to display next to articles, as take action boxout do. 

Tasks:

1. in the take action boxout, include option to select between Take action Page and EN form
2. change text from 'Select Take action page' to 'Select Take action Page or EN form'
