As a user I'd like to see the labels of form fields when I start filling them in.

It's a usability/accessibility best practice to have a label above the fields. Currently we only have a placeholder text that disappears when the user clicks in the field.

If it's not too complicated, I'd like to have the placeholder text to use a transition to become a label, which is a great solution to keep the UI streamlined.

This is not a priority ticket, if the implementation takes more than 4 hours then we can drop it.
