beside the Form itself, we must review the functionality, because based on the option selected (e.g. update the the Home Address), an in-page functionality gets triggered to allow self-service use cases for the user (like changing his bank account).

An e-mail is sent to the P4 admin on submit (I suppose this triggers a Supporter Care), this is really cool and shoukld be uynder our radar for "Activist page" and / or personalisation.
