Engaging Networks plugin: Add radio button for selection of template

      In the same way that in the current submenu block the editor can select style, we want to be able to select the style in the form block:

      The task is to add two radio buttons with thumbnails for the two (currently) styles we want to present the form. (The one will be to replace the current happy point, the other will be for a form inside the body of a post).
