Some style elements are missing:
1) List styles, when using e.g. <ol> or <ul>
2) Styling of “Checkbox”-type Opt-in form fields seems to be wrong (e.g. text is not in the same line as the box), however its OK in case of Question fields.
3) Label of “Radio”-type font fields should be displayed, since now only the answers are visible to the user, the question is hidden.
4) The optional dark blue background on column 2 is a bit too dark (e.g. blue Radio buttons are hardly different in color from the background), maybe worth considering a more transparent background (0.3-0.5 instead of 0.8)
