EN form - make "Content Title" and "Content Description" only available for "Form on side" style

As an admin, i need to see the fields:

• Content Title – The title you want to appear on the left-side of your Form
• Content Description – The description you want to appear on the left-side of your Form

ONLY if I select the Style 3 - Form on the side - because these 2 fields are not relevant for the other 2 styles.

Now, whether we make them visible or not clickable is the same, we don't want to have those fields available in the other 2 styles, so they can totally be grayed-out, like the Background image when selecting the Style #1 (see snapshot)
